We’re thrilled to share some incredible news about the exceptional clinical performance of our health centers. Five Cabin Creek Health Systems locations have earned prestigious national recognition from the American Medical Association and American Heart Association for their outstanding work in blood pressure and diabetes management.

Gold+ Recognition for Blood Pressure Excellence

Four of our health centers have achieved the highest level of recognition in the Target: BP program:

  • Cabin Creek Health Center (Dawes)
  • Clendenin Health Center (Clendenin)
  • Kanawha City Health Center (Charleston)
  • Sissonville Health Center (Sissonville)

The Gold+ recognition is awarded only to organizations that achieve blood pressure control rates of 70% or greater while excelling across multiple evidence-based care pillars. This includes accurate measurement techniques and rapid response protocols that ensure our patients receive the best possible cardiovascular care.

This Gold+ recognition places these four centers among the top-performing healthcare organizations nationwide. As recipients, our health centers will receive recognition at American Heart Association national conferences and be featured on the Target: BP website alongside joint American Heart Association and American Medical Association press releases.

Gold Recognition for Diabetes Management

These five health centers also earned Gold Certificate of Recognition from the American Heart Association’s Target: Type 2 Diabetes program:

  • Cabin Creek Health Center (Dawes)
  • Clendenin Health Center (Clendenin)
  • Kanawha City Health Center (Charleston)
  • Riverside Health Center (Belle)
  • Sissonville Health Center (Sissonville)

This recognition highlights our impressive HbA1c control in patients with diabetes and demonstrates our commitment to managing not just blood sugar levels, but also the cardiovascular risk factors that often accompany diabetes.

A Team Effort That Makes a Difference

These achievements reflect the dedication and hard work of our entire care team. Under the leadership of Emma White, our Director of Quality and Risk Management, our providers, patient service representatives, and medical assistants have implemented evidence-based practices that directly improve patient outcomes.

What This Means for Our Patients

These recognitions represent real improvements in the health outcomes of the patients we serve. When we achieve 70% or higher control rates for blood pressure and diabetes, it means:

  • Reduced risk of heart attacks and strokes
  • Better long-term health outcomes for our diabetic patients
  • Lower rates of diabetes-related complications
  • Improved quality of life for our patients and their families

Our Commitment to Excellence Continues

At Cabin Creek Health Systems, our mission has always been to promote the health and well-being of all people in our communities, especially the most vulnerable. These national recognitions validate our approach to whole-person care that integrates primary care, behavioral health, and preventive services.

We’re proud of this achievement, but we know our work is far from over. We remain committed to continuous improvement and to providing evidence-based, compassionate care to every patient who walks through our doors.
