Position Summary
As a Family Nurse Practitioner, you will:
- Provide comprehensive primary, secondary, and tertiary care according to standardized protocols.
- Establish and maintain confidential medical records for each patient.
- Coordinate patient care including referrals and health education.
- Ensure high standards of care in compliance with CCHS policies and regulatory guidelines.

Essential Functions and Responsibilities
Assess patient health through interviews and physical examinations.
- Complete patient histories, perform physical examinations, and develop care plans.
- Organize and administer health services, ensuring effective delivery of patient care.
- Collaborate with the Chief Medical Officer on quality care issues.
- Serve as a resource and partner for community health initiatives.
Qualifications
- Current WV APRN Licensure.
- Strong ability to use electronic health records.
- Excellent communication skills, with a focus on compassionate patient care.
- Experience in managing complex patient cases and maintaining calm under pressure.
