This recognition demonstrates a system-wide commitment to delivering the highest quality care for our patients.

O’Kane. “Recognition shows that Cabin Creek Health Systems has the tools, systems and resources to provide its patients with the right care, at the right time.”

You don’t just see a provider when something is wrong. You have a team making sure your checkups, lab work, prescription and specialist referrals line up so that nothing important is missed.This also means we offer multiple ways to reach us: by phone, through our patient portal, and in person so you can connect with your care team in a way that works best for you.

- Coordinated, team-based care means your entire care team is operating from the same playbook. Our primary care providers work hand-in hand with your Care team, which consists of your CCHS pharmacist, transition of care coordinators who maintain your healthcare continuity when you have an outside hospital or specialist visit, care managers who help connect patients up for success on their healthcare journeys, and behavioral health staff² who can help with healthy habit-building.

- Patient involvement means visits aren’t one-sided where we only tell you what to do. We listen to your concerns, respect your preferences, and work with you to create care plans that fit your life. We are here to help you understand your conditions and treatment options in plain language. When you leave an appointment, you know what the next steps are, why they matter, and who to contact if you have a question.

We follow what the science says actually works
- Evidence-based quality care means we are using proven medical guidelines, not guesswork. This matters because treatments are chosen based on what research shows helps people live healthier, longer lives. When we recommend screenings, medications, or lifestyle changes, you can trust that these decisions are backed by science and tailored to your needs.
For example, if you have hypertension / high blood pressure, we know exactly which screenings you need and when. We track whether you are getting those preventative services and use reminders and outreach to make sure you don’t miss important care. We can identify and close any gaps in your care because we are systematically monitoring across all of our locations.
Your health information works for you
- Health information technology used for your benefit. Your medical records are available when and where you need care across all CCHS locations. Lab results, medication lists and visit summaries are accessible through your patient portal. If you need to see a specialist outside of our system, we can quickly and securely share your information with them.

NCQA is a private, nonprofit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations and manages the evolution of HEDIS®, the performance measurement tool used by more than 90 percent of the nation’s health plans.
The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinician practices are functioning as medical homes and to recognize them for these efforts.
Earning this recognition required our teams to undergo a rigorous, independent review of our clinical processes, patient care systems, and quality outcomes. It’s third-party validation that we’re delivering on our promises to you.
