What is a patient-centered medical home?

A “medical home” is a health center where a team of health professionals works together with you to ensure that services are available when you need them and to provide the care that best fits your medical situation and your values. Your Medical Home Team may include:

• Physician Associates (PAs)
• Nurse Practitioners (NPs)

• Dentists
• Pharmacists
• Health Coaches

• Behavioral Health Providers
• Patient Service Representatives
• Nurses and Medical Assistants

A Medical Home Offers Care for All Members of Your Family

Adult Care: We will keep track of important preventive care and vaccinations, and remind you when they are due. We are available for acute care and will help you manage chronic conditions such as diabetes, high blood pressure, heart, and lung diseases.

Integrated Behavioral Health: We provide a full range of behavioral health services, and our behavioral health providers work closely with patients, their family members, and their medical providers to help patients better achieve their goals.

Women’s Health: We offer women’s wellness services at low or no cost through the Family Planning and Breast and Cervical Cancer Screening programs.

On-Site Pharmacies: The pharmacist works closely with you and your clinician to manage medication safely and efficiently. We strive to make medication both affordable and convenient, with most prescriptions being filled within a half-hour of request.

Children’s Health: We will remind you when your child is due for well-child exams and immunizations, and we are available for urgent care and to manage your child’s chronic conditions. (Free immunizations are available for eligible children.)

Dental Care: Available for adults and children. For uninsured patients, charges are discounted based on income.

Prenatal Care and Obstetrics: Provided by a referred partner health organization. We’ll work with you and your prenatal / obstetrics providers for the health and wellbeing of you and your child.

If You Have a Chronic Condition

We will help you learn more about managing chronic conditions and expect that you will actively participate in making decisions about your care.

We will provide the information and support you need to take actions to improve your health and healthcare. Please ask if you have questions about how to deal with your condition.

Sometimes it is helpful to come together with others facing similar health issues. The following group sessions are offered periodically:

• Group Medical Visits for patients with chronic conditions

• Diabetes Education Programs

• A HEAL Group (Health Education for Active Living)

• A Nutrition and Weight Management Group

Smoking Cessation Counseling available by referral

OUR COMMITMENT

Your Care Team is available 24/7 if you need them. You can communicate with your care team by phone or via the patient portal, and you can get an appointment quickly, even on the same day when needed. If you need to speak to a provider after your clinic’s location is closed, such as an evening or weekend, call the CCHS after hours phone number, 1.800.789.0280.

Your Care Team knows you and your health history. We ask you to share your health concerns and goals as well as past medical services you have received.

Your Care Team helps you understand your health condition and how to take care of yourself. We help you sort through your options and make decisions about your care.

Your Care Team coordinates your care. We follow up if you receive hospital services, and we will coordinate care with your specialist physician. Please let us know when you see medical services outside of Cabin Creek Health Systems.

Improving Health Takes a Partnership

Patients are a part of the care team and are encouraged to actively participate in creating a healthcare plan. Your healthcare team will provide accurate information based on medical science as well as encourage your own self-care between visits. As partners, we will be available when you need us.

Eliminating Financial Barriers to Care

Let us know if cost is a barrier to healthcare for you. Cabin Creek Health Systems honors and bill almost all insurers, including Medicaid and Medicare. In addition, we can help you apply for the CCHS Sliding Fee Discount Program and other programs that cut financial barriers to care. These include:

  • Programs offered under the Affordable Care Act, including expanded Medicaid and private insurance programs
  • CCHS Pharmacy Discount Program (340B)
  • Medicaid and CHIP for uninsured children, pregnant women and others
  • The Family Planning Program
  • The Women’s Breast and Cervical Cancer Screening Program (BCCSP)
